KVKK Application Form

According to Article 11 of the Personal Data Protection Law No. 6698 (“KVK Law”), personal data owners, defined as related persons (“Applicants” hereinafter), have the right to make requests regarding the processing of their personal data. According to the first paragraph of Article 13 of the KVK Law, applications related to these rights must be submitted to our Company, which is the data controller, in person, in Turkish, in writing, and by the methods determined by the Personal Data Protection Board ("Board").

Below are the details on how written applications can be submitted to us.

According to the Communiqué on Procedures and Principles for Application to Data Controller (dated 10/03/2018 and numbered 30356), the following information must be included in the applications:

Information to be Filled During the Application:

• Name, surname, and signature if the application is written,
• Turkish ID number for Turkish citizens, nationality, passport number, or identity number if applicable,
• Residential or workplace address for notification,
• If applicable, electronic mail address, telephone and fax number for notification,
• Subject of request,
• Additionally, information and documents related to the subject are attached to the application.

If you wish to make an application regarding your personal data, the application form below must be filled out completely. This application form has been regulated to determine your relationship with our Company, to accurately identify the personal data processed by our Company if any, and to enable us to respond to your application correctly and within the legal period. To mitigate legal risks arising from illegal and unjust data sharing and especially to ensure the security of your personal data, our Company reserves the right to request additional documents and information (copy of ID card or driver's license, etc.) to identify and authorize. The Company does not accept liability for requests arising from incorrect information or unauthorized applications.

Applications submitted to us will be responded to in writing and free of charge within thirty days from the date the request is received, depending on the nature of the request, as required by the second paragraph of Article 13 of the KVK Law.
